Kids on Wheels Waiver and Photo Release Form

The Kids on Wheels Camp involves children riding balance bikes in an enclosed area with instruction and supervision from volunteers and staff of the British Columbia Cycling Coalition (also referred to as the BCCC in this document). HEREAFTER REFERRED TO AS THE “CAMP”

I hereby acknowledge and agree that: participation in a bicycling activity, including the Camp, may be dangerous, exposing participants to some risks and hazards.

I hereby give my permission for my child to take part in the Camp as outlined above, and hereby agree to well and sufficiently indemnify and save harmless the BCCC and keep the BCCC indemnified and harmless of any and all demands, actions, proceedings, liability claims, damages, together with the costs and expenses thereof, that may hereafter at any time be made or brought by or on behalf of my child for any injury, loss, damage, expense and costs sustained or alleged to have been sustained by my child howsoever arising from the aforesaid Camp provided.

By filling out and submitting this form, I confirm that I am the legal parent/guardian of the child and give full permission for their participation in the Camp under the conditions outlined above.

Photo and Video Consent and Release (optional)

I hereby give consent for representatives of the British Columbia Cycling Coalition (also referred to as the BCCC in this document) to photograph and/or publish photographs and videos of my participating child.
Kindly note, under no circumstances will the BCCC sell photos/videos to third parties nor reveal identifying information or the names of subjects in the photos/videos without further consent.

By filling out and submitting this form, I confirm that I am the legal parent/guardian of the child and give full permission (or refusal) for photo and media use under the conditions outlined above.
